How to self-refer and book your appointment
Book a standard physiotherapy virtual appointment – 45 minutes
The initial assessment is normally conducted over the phone and will aim to provide you with a diagnosis and treatment plan. A follow-up (either face to face or telephone/video) will usually be arranged one to two weeks following the initial assessment to ensure progress is being made. At this point discharge will be considered allowing you to continue to self-manage your condition.

If you would like to attend your initial appointment in person then please book your appointment via our booking service and add the words (face to face) instead of a telephone number.

Before your appointment
To help check your progress over time we use a short questionnaire. This records your symptoms and quality of life in a standardised way and means particular aspects of musculoskeletal health can be discussed in the appointment.
We’ll ask you complete this questionnaire on the day of your first assessment and again after you’ve been discharged.
