I am requesting the Regulation (EU) 2016/425 Declarations of Conformity for all type IIR surgical masks provided as PPE to staff since 15 June 2020.
Regulation (EU) 2016/425 states that the Declarations of Conformity must be provided physically with the PPE or a weblink printed on the PPE packaging. Which means if the PPE provided is legally complaint, then the Declarations of Conformity will be readily accessible.

Under Section 1(1)(a) of the Freedom of Information Act (FOIA) the Trust can confirm that it does not hold the information requested.
All Type IIR masks were provided to through the national central push stock model over this period. Information about this programme can be found on the NHS England website
For this reason we do not hold the information you are seeking and therefore this information is not available under the terms of the Act at this time.
