Community Neurological Rehabilitation Team – North

This service provides assessment and therapy to patients in the north of West Sussex who have any neurological diagnosis, to maximise their independence and manage their condition.

The Community Neurological Rehabilitation Team – North is part of Sussex Community NHS Foundation Trust.

We are a team of professionals working together to help people who have been clinically diagnosed with a stroke, subarachnoid haemorrhage or acquired brain injury within the last 12 months, or with a diagnosis of a progressive neurological condition and who have the potential for improvement through rehabilitation.

Our team will work with the patient and their families to help them manage at home and in the community either through rehabilitation or resettlement/support.
