Learning disabilities liaison team

Our learning disabilities liaison team provide care and treatment that meets your needs. This includes making any reasonable adjustments.

The team help people by:

• offering longer appointment times
• supporting them in understanding their treatment plan
• preparing them for hospital visits or stays
• supporting hospital staff to communicate well.

We also encourage you to bring any tools that you use such as communication boards with you to hospital.

My Care Passport

We use a form called ‘This is me – my care passport’ to help you or your carer to tell us important information about you and your needs. We ask you to keep the form updated each year.

If you would like to discuss any communication or other needs that you may need additional support for, please contact the ward or department (in person, by email or telephone) using the contact details on your appointment or admissions letter.
