The Worthing and Southlands cardiac rehabilitation service is certified by the British Association Cardiac Prevention and Rehabilitation (BACPR) and run by a specialist team of physiotherapists, nurses and practitioners.
This means we can provide a structured programme tailored to the individual needs of every patient. Together we will work with you to manage your heart condition.
Support when you leave hospital
You may have been visited by a member of our team while you were in hospital. If you were not seen during this time, you will be referred to our service, and a member of the team will contact you by telephone to offer advice and support.
Assessment clinics
Once referred to the service, you and your family are invited to attend a one-to-one clinic appointment or a telephone appointment with a member of the team to assess what help you may need in your recovery and to answer any questions you may have.
You may also undergo a supervised walking test to check your level of fitness.
Exercise
You will be given the opportunity to come to a course of weekly exercise sessions to help gain the knowledge, confidence and motivation to carry on exercising for life.
Alternatively, if you can’t attend the exercise sessions or prefer the convenience of exercising at home, then we can offer a home-based service and / or online links to exercise videos. A more individual programme based on your needs may also be provided. These will be followed by telephone calls from a member of the team to further support your progress.
Information/education
We have recorded an educational series. These will give you more information about your heart and how to help yourself.
We also provide a wide range of written information and leaflets for patients and their family to take away with them.
